Independent denial recovery serviceARIZONA / SPECIALTY PRACTICES

Your denied claims,
worked and recovered.

Payer Delta Arizona works the commercial denials your team hasn't had time to appeal. We prepare the appeal, you sign and send it, and you pay 22% only on money that actually comes in. If nothing is recovered, you pay nothing.

The delta is the gap between what you billed and what you were paid. We work to close it.

ILLUSTRATIVE WORKFLOWNot a client result

Review the denial, prepare the appeal, and track any payment.

01 / FIND THE OPPORTUNITY

We review your
unworked denials.

In this sample, a spine practice receives a commercial-plan denial. Review the remittance, the denial notice, and the plan requirements before the payer’s appeal deadline. Missing facts stay visible while the next action is assessed.

YOUR DELIVERABLE

A prioritized denial inventory with source and deadline questions visible.

02 / PREPARE THE NEXT ACTION

We prepare
your appeal packet.

Match the applicable commercial plan policy to the service, date, and supporting records your team authorizes. Resolve gaps before preparing ready-to-sign appeal packets for your practice’s final review.

YOUR DELIVERABLE

Ready-to-sign appeal packets after supporting-record and review checks are complete.

03 / FOLLOW THROUGH

You sign and send.
We track the result.

Your practice reviews, signs, and submits. Track the payer’s response, then reconcile any later remittance before calling money recovered. This illustrative example claims no successful appeal or collected payment.

COMMERCIAL DENIAL RECOVERY

You pay after the money comes in.

Your practice receives payer payments directly. We invoice 22% of recovered money under the agreed terms. If nothing is recovered, you pay no commercial recovery fee.

MEDICARE PACKET PREPARATION

$95 / administrative packet

A separate flat-fee scope, confirmed before work begins. Commercial contingency terms do not apply to this offer.

ONGOING REVENUE INTEGRITY & RCM

An optional monthly plan.

Ongoing RCM support is $1,500–$3,500 per month, depending on the agreed work. Payer intelligence and cash forecasting can be included in a separate proposal.

Service availability, responsibilities, inputs, and fees are confirmed in writing. Outcomes and payment timing depend on the underlying claims and payer decisions.

OPTIONAL ONGOING SUPPORT

Help your billing team
prevent repeat denials.

After recovery:
keeping denials from coming back.

Once the backlog is worked, you can choose ongoing help with recurring denials, payer changes, and cash reporting. These services are separate from commercial recovery.

01 RCM support for recurring denials

Review repeat denial reasons, missing documents, and billing-team handoffs. Assign the next action to the right person in your existing workflow.

02 Payer intelligence

Follow relevant payer policy and reimbursement changes, with source links and regular briefs. Bring updates to the team before the same issue affects more claims.

03 Cash forecasting

Review aging receivables and appeal progress in a 30-, 60-, or 90-day cash outlook. Keep assumptions separate from actual collections and compare the forecast with what gets paid.

Questions
before you reach out.

Does Payer Delta replace our billing team?

No. The recovery, RCM support, intelligence, and forecasting scopes are designed to work with your practice or billing team. Your agreement defines the work each party owns.

Who signs and submits a correction or appeal?

Your authorized practice team reviews the prepared materials and owns signing and submission. Missing sources, supporting records, or authority need to be resolved before that handoff is ready.

What should I send in the first inquiry?

Start with your practice name, your role, and an estimate of your unworked commercial denial backlog (approximate dollar amount and primary payers). Please do not send patient records, Social Security Numbers, or raw claim exports through the web form. Data-handling requirements, any necessary agreements, and a suitable secure transfer process are confirmed before remittance files are requested.

Is there one appeal deadline for every claim?

No. Filing requirements depend on the payer, plan or product, type of action, review stage, and applicable dates. For Original Medicare fee-for-service, a first-level redetermination generally must be requested within 120 days after receipt of the initial claim determination. Commercial plan requirements must be checked for the specific claim. Review the applicable notice before the payer’s appeal deadline. CMS first-level appeal guidance ↗

Does the 22% fee apply to every service?

The 22% fee applies to commercial denial recovery. Medicare packet preparation and optional ongoing RCM, payer intelligence, or forecasting services are priced separately and agreed before work begins.
